You can request a baptism certificate by filling out the Sacramental request form (listed below) and emailing it back to our parish office at [email protected].
Please be mindful that our processing times vary per season and normally take 2-3 business days to have your certificate ready for pickup.

IMPORTANT: Only complete the baptism form if you were baptized at St. Mary's in Hood River. If you were baptized at another parish, you will need to contact them for a copy of your baptismal certificate.
